Tenant Service Coordination

The Tenant Service Coordination program is a service that supports recovery through assisting individuals with significant mental health challenges to successfully obtain and maintain their permanent supported housing. The service supports individuals through the life cycle in assuring that permanent housing is maintained and that any housing-related issues are addressed in a timely manner in an effort to support independence. The program assures to promote continuous quality improvement and progression toward consistency of service delivery and to improve outcomes for individuals.
